Buy a Muni map and Muni Pass http://www.sfmta.com/cms/mfares/passes.htm then you're good to go. SF public transpo + a lot of walking > driving a car in SF (parking is expensive)

Union Square - to chill and watch people
Westfield SF - shopping, lost of yummy restos in the Food Emporium
Chinatown - rice toppings a must try (roast pork, peking duck, soy chicken, etc)
Pier 39 / Fisherman's Wharf - look for Bush Man, avoid the wax museum. Joe's Crab Shack, Johnny Rockets and In-n-Out FTW! Clam chowder in sour dough bread bowl is a must try for first timers.

Fillmore St - unique shops, restos, bakeries and cafes for http://www.fillmorestreetsf.com/. I love the Marc Jacobs shop!
Tenderloin - walk the streets of Tenderloin for some thrills. Okay I don't suggest you to do this unless you want to be harassed by crackheads and drug dealers. Ha ha.
Coit Tower - great view of the city
Golden Gate Bridge - walk the whole stretch of the bridge and back
Alcatraz - Night tours are the best!
Japanese Tea Garden / Golden Gate Park
Crazy Horse - look for Bunny.
Gordon Biersch - thin crust pizza + beer = yum
Golden Boy Pizza, North Beach - best pizza squares, open late night. No, check that. BEST SF PIZZA.
Mighty SF
Taqueria El Sol - killer Chimichangas
AT&T Park - watch a Giants game!
Different art galleries around Union Square
Cable car ride
Lombard St
The Hundreds, Huf (closed recently), Shiekh, First Step, Nike Town - all within a few blocks from Union Square
Darkside Initiative - a local streetwear store fav
Haight St - more shops
Japantown - Make sure you check out the mall. It doesn't look much from the outside but once you go in, you'll see a variety of shops and restos. Try Sophie's Crepes, Kinokuniya Bldg. Also visit Fatlace
Little Italy - nice place to chill and enjoy a bottle of good wine after dinner.
Cheesecake Factory (for an awesome view of Union Square)
Rent a GoCar http://www.gocartours.com/sanfrancisco.html turn off the GPS and drive til you get lost!

If you're gonna be in SF for at least 5 days, I suggest you buy a City Pass http://www.citypass.com/city/sanfrancisco.html. It's worth every penny! I say at least five days because it'll take you a day to visit each attraction.
